| Summary of Issue | 2026 Update for Countersunk Screws being cracked or loose is an out of service condition. Why is there no wording in regards to them missing? |
| Justification or Need | If one countersunk screw for the upper coupler assembly is broken or loose automatically out of service, how is one missing not? I understand if there is a bolt hole on the lower or upper plate with no corresponding bolt hole, then no violation or out of service condition should be applied. But if there is a corresponding bolt hole on both the upper and lower plates, then a violation and out of service condition should be applied. |
| Request for Action | Add the word "missing" in the out of service criteria under Sec. 3(b)(5) - Upper coupler assembly and add guidance in Op policy 15 advising that there is only a violation and out of service condition when there are corresponding bolt holes on both plates. |
