| Justification or Need | Although these issues have been addressed in the past I have never had them arise in a fatal crash situation. The above combination of issues resulted in and caused a fatal crash. |
| Request for Action | The first two issues should be inserted into the OOS Criteria. Parking brakes should never have been taken out of the OOS. The 3rd issue should be discussed and automatic slack adjusters that go out of adjustment should be defined as defective therefore requiring a repair or replacement of the defective component. The issue for discussion will be how to conclude that the AS is the component at fault. |
