| Summary of Issue | Cargo secured with Monkey Grip tiedowns. Tie downs were not long enough to anchor both side of the trailer. So the compagny used a flat hook tiedown and hooked it to a normal hook tiedown to secure the load. As soon as any loose gets into the tiedowns easily able to slide the hooks out of each other. |

| Request for Action | Would need clarification on what is an acceptable anchor point for a tiedown. If alternated Anchor points (Other than on the trailer or machinery is used, provide guide lines on what not to do. |
