| Summary of Issue | It is our understanding that when a vehicle meets the out of service criteria and is placed out of service that an out of service sticker is to be affixed to the vehicle. This policy is noted in Op Policy 5 but we understand that this practice is not always carried out. Our question is whether or not it is CVSA's desire for this policy to still be adhered to? If not, should Op Policy 5 be revised? 4. Applicable Forms • A Driver-Vehicle Examination Report is to be used to report the results of driver, vehicle, and cargo examinations. It is to be prepared even though no defects are discovered and a copy given to the driver regardless of whether or not the driver consents to sign the form. • A vehicle out-of-service sticker is to be affixed to a vehicle that has been declared out-of-service. • CVSA decals are to be affixed to a vehicle that passes inspection, as described below. |
| Justification or Need | This is not an immdeiate need. We were wondering if this policy is current as we are due to order more OOS stickers if it is. |
| Request for Action | Review and updating of Op Policy 5 if necessary. |
