| Summary of Issue | Wheel, tire and axle assemblies on modular home trailers being maintained improperly causing failures of the axles. |
| Justification or Need | These wheel, tire and axle assemblies are being reused (over used) based upon the axle manufacturer's (Dexter Axles) requirements. The over use causes axle failure (bearings/wheels, etc.) and these failures have lead to a fatality in Hooksett, NH. The manufacturer states the axles are single use and only good for 1,000 miles. |
