Regional COHMED Room Reservation Request Form

Please use the form below to make a reservation request for the upcoming COHMED Regional Training being held in Columbus, Ohio on June 12-13, 2024. The deadline to request a reservation is May 27. Lodging will be direct billed to CVSA. Participants must use the form below to book their stay at the Plaza Hotel Columbus at Capital Square. Reservations made outside of the CVSA room block will not be reimbursed.
